You will need to submit the following with your application:

1. A copy of your birth certificate, most updated picture ID card, or passport.
2. A notarized copy of your 8th grade (or higher) report card showing successful completion.
3. An essay/letter of intent outlining why you wish to enroll in this program. Please find the instructions here.
4. One letter of recommendation which must be sent directly from your teacher, advisor, mentor or any other adult (not a relative), who can attest to your academic abilities and personal strengths. The letter should be sent directly to registrar-ISHSC@mishkahu.com

In addition, you will need to:

1. Understand and comply with all the University's technology requirements found here.
2. Comply with all other University registration requirements.
3. If applying for more than one child, fill out a separate application and pay all fees for each child.

I understand that I am not entitled to copy, duplicate, access or otherwise appropriate such proprietary information for the benefit of any third party.

I hereby apply to Mishkah and certify that to the best of my knowledge the information given in this application is true and correct. I understand that all items submitted to Mishkah as part of this application process become the permanent property of Mishkah University and will not be returned. If the Admissions Team is notified that any information contained on this application is false, it will be grounds for immediate dismissal.

I understand and accept full responsibility of any legal consequences resulting from forged/fake information/documents.

I further understand that Mishkah University maintains the right to terminate my enrollment should I breach any of the above stated standards and policies.
